Personal Information Collection Statement

This statement explains how the Hong Kong Aircrew Officers Association (HKAOA) collects and handles the information you provide when you complete this survey. Please read it before you take part. Who is collecting your information The Association is the Hong Kong Aircrew Officers Association. If you have any question about this statement, or about how your information is handled, you can contact the Association at [email protected]. Taking part is voluntary Taking part in this survey is entirely voluntary. Every question is optional and includes a "Prefer not to say" option, and you may stop at any point before you submit. If you close the survey without submitting, no answers are recorded. What information is collected If you choose to answer, the survey collects your responses to its questions. These cover your experience of, and views on, the current sick leave and absence management arrangements, and they include your rank, your years of service, your HKAOA membership status, and an optional free-text comment. Some of this information relates to your health and to decisions you have made about your fitness for duty. Separately, to confirm that only invited pilots take part and that each invitation is used only once, the survey checks a single-use invitation code and a staff identifier that you enter. This check is handled separately from your answers, as described under "How your anonymity is protected" below. Why it is collected and how it is used The Association collects this information to understand how the current sick leave and absence management arrangements affect pilots' fitness-for-duty decisions. The information is analysed and reported only in aggregate. Individual responses are not reported. The Association will not use your information for any purpose other than those described in this statement. Who your information may be shared with The aggregated findings of this survey may be shared with regulatory authorities, with the Association's professional advisers, and with bodies to whom the Association makes representations in connection with the matters described above. Aggregated findings do not identify individual respondents. Any free-text comment is reviewed before any external use, and is not shared in a form that could identify you or any other person. How your anonymity is protected The survey is designed so that your answers cannot be linked to you. The record that you have taken part is kept separately from the record of your answers, and the two are designed so that they cannot be joined. Dates, not precise times, are recorded, so that the two records cannot be matched by timing. The staff identifier you enter is used only to confirm your invitation and is not stored with your answers. The free-text comment is the one place where you could unintentionally identify yourself. Please do not name individuals, identify specific dates or flights, or include anything that could identify you or another pilot. Free-text comments are reviewed before any use. How your information is kept secure Your information is held securely, and access is limited to those who need it for the purpose described above. How long your information is kept The Association keeps your information only for as long as it is needed for the purpose described in this statement. In practice: * your survey responses are retained until the conclusion of the Association's work on the matters described above, and in any event no longer than 2 years from the close of the survey, after which they are deleted; and * any free-text comment is reviewed, and once its content has been recorded in aggregated form, the original comment is deleted. Your right of access and correction Under the Personal Data (Privacy) Ordinance, you have rights of access to and correction of personal data held about you. Because this survey is designed so that your answers cannot be linked to you, the Association is, by design, unable to identify which response is yours in order to retrieve or amend it.
